Notaries are not allowed to choose the type of form to be used in a document notarization. Typically, the document will come pre-printed with the form needed. If it does not, you will need to choose which type of notarization form you need. CM Notary Service can provide both California Acknowledgement and Jurat forms if needed.
The purpose of an acknowledgement is for a signer, whose identity has been verified, to declare to a Notary that he or she has willingly signed a document.
The purpose of a jurat is for a signer to swear to or affirm the truthfulness of the contents of a document to a Notary.
